John Terry lifted the Premier League trophy in front of the Stamford Bridge crowd

Chelsea beat Sunderland 3-1 on the final day of the season before lifting the trophy as 2014/15 Premier League champions.

Stephen Fletcher put the visitors in front as the party looked like it might be spoiled, but a Diego Costa penalty and a Loic Remy double gave Jose Mourinho's men a 3-1 win at Stamford Bridge.

Chelsea sealed their first league title in five years when they beat Crystal Palace 1-0 to take an unassailable lead in the table with three games remaining.

They lost to West Brom on Monday but finished the campaign with a win before captain John Terry lifted the trophy for the fourth time.
