Jack Butland is facing a further scan on his ankle after pulling out of Stoke's U23 side just before kick-off on Friday night.
England goalkeeper Butland was due to play for the Potters' second string against Reading, ending an absence that dates back to March.
Butland fractured his ankle making a clearance while playing for England against Germany and he was unable to start the season on time after suffering a setback over the summer.
However, he was named in Stoke's starting line-up for the Premier League Cup clash at the Madejski Stadium, a game Reading won 1-0, but had to be replaced by youngster Harry Isted.
Speaking after Stoke's 2-0 Premier League win over Burnley, boss Mark Hughes said: "It's a little bit of a concern, I have to say.
"He got to the warm-up (on Friday) and felt discomfort in the ankle, the same situation that we found ourselves before the beginning of the season, unfortunately. Not to the same extent, but in the same area, so we need to get to the bottom of it.
"I haven't had an update in terms of where we think he is with it but he's gone for scans. Hopefully we can settle it down again and continue to try and get him ready for games again.
"If that's not the case then we'll have to try and do something more radical, maybe, but we'll try and avoid that if we can."
