West Ham will meet with Carlos Bacca's representatives this week to discuss personal terms over a club record move to the Olympic Stadium, according to Sky sources.
The Hammers have activated the £26m release clause in the forward's current contract at AC Milan and will now hope to persuade him to join the club.
If terms can be agreed, Bacca would become Slaven Bilic's sixth signing of the summer following the arrivals of Gokhan Tore, Sofiane Feghouli, Toni Martinez, Havard Nordtveit and Domingos Quina.
The Colombia international would also become the most expensive purchase in West Ham's 121-year history, exceeding the £15m spent on the signing of Andy Carroll from Liverpool in 2013.
Bacca, 29, joined AC Milan from Sevilla only last summer and enjoyed a superb first season in Italy, scoring 20 goals in 42 appearances.
He scored 49 goals during two seasons at Sevilla, helping the Spanish side win the Europa League in 2014 and 2015.
West Ham co-owner David Sullivan told Sky Sports News HQ in May he was confident of securing a "top class forward" ahead of the club's first season at the Olympic Stadium, but has since failed with moves to sign Alexandre Lacazette, Michy Batshuayi and Callum Wilson.
