Sky Sports will show the Premier League matches involving Liverpool and Manchester City exclusively live as the race for the top four goes down to the final day.
Liverpool must beat Middlesbrough at Anfield to guarantee Champions League football next season and you can see the match live on Sky Sports 1 from 2pm on Super Sunday.
City must pick up a point in their final match against Watford at Vicarage Road to secure a top-four spot and that game will be on Sky Sports 2 from 2.45pm. Both matches will be available in Ultra HD.
Furthermore, Sky Sports customers will also be able to see in-game clips and highlights from all 10 of Sunday's Premier League matches on their mobile devices via the Football Score Centre app.
That means you can see goals from Arsenal v Everton, Manchester United v Crystal Palace, Hull v Tottenham and all of the other matches within moments of the ball hitting the back of the net.
You'll also be able to follow all of the drama of the final day with Jeff Stelling and the panel on Soccer Special on Sky Sports News HQ.
Plus Sky Sports viewers in Republic of Ireland will be able to see champions Chelsea conclude their league season against Sunderland, before John Terry and Gary Cahill lift the Premier League trophy.
All Sky Sports viewers will be able to see the trophy presentation as we base our coverage at Stamford Bridge on the day.
