The pick of Swafford's late run came at the penultimate hole, where a stunning tee-shot left a tap-in gain to move ahead of nearest challenger Adam Hadwin.
Hadwin fired 13 birdies during a third round 59, with his putt at the last securing only the ninth sub-60 round in PGA Tour history.
Brian Harman was also in the title hunt during an enthralling final day, where he gain the early initiative by draining a 65-footer at the sixth hole.
In the Middle East, Tommy Fleetwood ended his three-and-a-half year wait for a second European Tour title - where a hole-out eagle set him on his way to a one-shot win.
It was also a special round for 2014 champion Pablo Larrazabal, who chipped in from off the green to stay in contention over the closing stretch.
