The Exiles bounced back from a 68-19 hammering by Stade Francais on their European debut last week to win by the narrowest of margins in the Pool 5 encounter in northern Italy.
The hosts scored four tries to the visitors' three to lead by two points with just four minutes to go before Exiles full back Alex Davies struck a 78th minute penalty to clinch victory for his side.
Hooker Dan George and wing Joe Ajuwa crossed in the first half, while Gordon Ross slotted a conversion and two penalties to give Welsh an 18-10 lead at half time, - with a try from Denis Mastriviic plus five points from the boot of fly half Riccardo Della Rossa providing the points for the hosts.
Ajuwa extended the lead soon after the break with his second but the Tuscan outfit hit back with converted tries from replacements Billy Ngawini and Niccolo Tempestini to make it 24-29 going into the final 10 minutes.
Lock Giacomo Bernini levelled the scores five minutes later and Marco Frati converted to give the Italians the lead for the first time.
But with just two minutes to go Davies broke Italian hearts with his second penalty of the half to grab victory for Lyn Jones' men.
