Ben Watson gave Exeter a first-half lead as he bundled the ball home from an Emmanuel Panther cross, but the striker went from hero to villain as he then missed a penalty for City 12 minutes after the break.
Watson earned the spot-kick after being fouled by Marc Joseph but saw his penalty superbly saved by David Stockdale down to his left.
Rotherham then equalised with a penalty of their own when Exeter goalkeeper Paul Jones was adjudged to have fouled Richie Barker and substitute Reuben Reid netted from the spot.
Exeter could have won it late on but Ryan Harley's header was kept out by Fulham loanee Stockdale, who made another outstanding save to earn his side a point.
